Assess Your Knowledge

This section of the Residential Aged Care Toolkit provides an opportunity to:

  • evaluate your learning and development needs in providing palliative care and advance care planning for older people, and 
  • create a personal learning plan.

Suggested resources are below that can help improve your knowledge, skills and confidence in providing palliative care and advance care planning.

Personal Learning Assessment and Plan

The ELDAC Personal Learning Assessment (PLA) is an interactive form to assist in identifying your personal learning and development needs. It is recommended to complete this assessment annually to review how your learning priorities have changed. The assessment has two sections:

  • Knowledge of palliative care and advance care planning
  • Skills and confidence in providing palliative care and advance care planning.

After completing your PLA and identifying areas where further training is required, it is recommended that you create an ELDAC Personal Learning Plan (PLP). Start by reviewing the items from your PLA where you self-rated as having no knowledge, and/or skills and confidence. From those items, select the topics you would like to focus improving on. Ensure your focus is realistically achievable over the next year.

The interactive ELDAC Personal Learning Plan follows the same format as the PLA. There are two sections on knowledge, and skills and confidence. The plan is divided into these two learning priority sections as these areas may be at different levels of achievement. For example, you may know the principles of advance care planning, but need more confidence in starting a conversation with someone about making an advance care plan.

Both the ELDAC Personal Learning Assessment and Plan can be used to discuss further training in palliative care and advance care planning with your supervisor.
