HealthPathways
HealthPathways is a web-based information portal, designed to be used at the point of care by primary care clinicians managing people in the community. [1] Pathways are identified and defined by the needs of people who share a particular symptom or condition, and consist of clear and concise guidance through clinical and referral information, and resources.
Each pathway is evidence-informed, but also reflects local reality, and aims to preserve clinical autonomy and individual choice. HealthPathways achieves this by engaging local health professionals to collaboratively develop and refine content.
Every Primary Health Network (PHN) has its own HealthPathways site.
See Find my HealthPathways below for the local HealthPathways site available in your region. Access to each HealthPathways site is restricted to that HealthPathways region’s catchment.
The HealthPathways Community enables HealthPathways teams across Australia, New Zealand and the United Kingdom to share knowledge, processes, pathways, and infrastructure.

Health practitioners can and are encouraged to participate in the development and refinement of their local HealthPathways by:
- suggesting a clinical area or condition for a new pathway by emailing your local HealthPathways project team
- becoming involved in clinical working groups on HealthPathways by emailing your local HealthPathways project team
- providing feedback on the HealthPathways site using the "send feedback" button (top right corner of each pathway).
Content is developed collaboratively by Clinical Editors, Clinical Leads and clinical working groups comprised of local general practitioners, hospital clinicians, and a wide range of other health professionals, through a process which documents guidelines for conditions within the HealthPathways.
