ELDAC Digital Dashboard
Unlike most other types of care, palliative care is distinctly different in that it seeks not to prolong life, but to achieve a good death through careful and considerate preparation in understanding care needs. To ensure holistic, person-centred care that drives the outcomes (a good death) and experiences that matter to the older person and their family are achieved, a digitally enabled and data-informed approach to decision-making at all levels is required.
The ELDAC Digital Dashboard (the ‘dashboard’) was developed to help support this in practice, making use of the ELDAC Care Model to provide prompts and triggers to guide care across the palliative pathway. The dashboard aims to:
- Support a consistent and comprehensive approach to end of life
- Show what is happening (client level, manager level, organisation level)
- Provide triggers and flags for care prompts
- Assist in reporting and/or benchmarking.
