During 2021-2024, 93 services, both Home Care and Residential Aged Care, have enrolled in the Linkages Program supporting service development by evidence-based strategies designed to achieve service improvements in palliative care and advance care planning and enhance linkages between aged, primary and specialist palliative care providers. The Showcase is both a shared learning experience and an opportunity for participants to celebrate their positive program outcomes achieved at the culmination of their ELDAC Linkages journey.
Program participants, partners and program delivery team were inspired on the day by services who presented on their ELDAC Linkage experiences. The impact of the service delivery improvements and benefits to services, workforce and older Australians receiving care was highlighted and emphasised the purpose of all ELDAC activity. Sharing stories and lessons learned revealed how providers across the country are creating connected communities of Palliative care champions and working towards delivering person centred care for residents, clients and their loved ones. The day was also a great opportunity for stakeholders to network and identify new opportunities for ongoing continuous improvement initiatives. An interactive session led by Program Leadership allowed for group discussion and reflection on which tools, resources and strategies will be needed to meet the new strengthened standards.
