How can the ELDAC facilitators support my organisation?
The ELDAC facilitators will:
- provide information about where to access resources to support delivery of evidence-based palliative care and advance care planning services for older Australians and their families
- assist to build or enhance local and/or regional linkages between aged care, specialist palliative care, primary care and other local service providers and networks
- provide advice on organisational strategies to create/enhance quality palliative care and advance care planning services.
What are the benefits of working with the ELDAC facilitators?

- positive palliative care outcomes for all older Australians and their families
- improved connections and communication pathways between aged, primary care and palliative care services
- increased staff confidence, skills and knowledge about palliative care, end-of-life care and advance care planning

- greater access to resources, knowledge and information sharing opportunities to support palliative care and advance care planning
- prevention of avoidable hospital admissions and emergency department visits for treatment of end of life issues
