The ELDAC Home Care Toolkit is designed for clinicians and care workers providing palliative care and supporting advance care planning for older Australians living with advanced life-limiting illnesses, their families and carers. The Home Care Toolkit is user-friendly and has been developed and reviewed by experts in aged care and palliative care.
The Home Care Toolkit has evidence-based resources on palliative care and advance care planning that can assist you and your organisation in meeting the Strengthened Aged Care Quality Standards.
