Linkages Program

Supports services to navigate resources and build partnerships using evidence-based strategies.

ELDAC facilitators describe the linkage process between specialist palliative and aged care providers and how this helps provide quality end-of-life care.

A team of facilitators support service development through the Linkages Program (formerly known as Working Together Program), navigation of resources, building partnerships, connecting with peak bodies from aged, primary and specialist palliative care.

The ELDAC Linkages Program is a facilitated palliative care and advance care planning initiative that aims to improve the quality of end of life care for all older Australians receiving aged care. The program is underpinned by evidence-based strategies to enhance linkages between aged, primary and specialist palliative care providers and is supported by the Linkages Toolkit.

How can the ELDAC facilitators support my organisation?

The ELDAC facilitators will:

  • provide information about where to access resources to support delivery of evidence-based palliative care and advance care planning services for older Australians and their families
  • assist to build or enhance local and/or regional linkages between aged care, specialist palliative care, primary care and other local service providers and networks
  • provide advice on organisational strategies to create/enhance quality palliative care and advance care planning services. 

What are the benefits of working with the ELDAC facilitators?

Benefits of working with the ELDAC facilitators include:

  • positive palliative care outcomes for all older Australians and their families
  • improved connections and communication pathways between aged, primary care and palliative care services
  • increased staff confidence, skills and knowledge about palliative care, end-of-life care and advance care planning
  • improved access to specialist palliative care advice and support
  • greater access to resources, knowledge and information sharing opportunities to support palliative care and advance care planning
  • prevention of avoidable hospital admissions and emergency department visits for treatment of end of life issues
  • shortened hospital stays, enabling the older person to return to their place of residence sooner
  • improved pain and symptom management for older Australians at the end of life.
