The ELDAC Residential Aged Care (RAC) Toolkit is designed for clinicians and care workers providing palliative care and supporting advance care planning for older Australians living with advanced life-limiting illnesses, their families and carers. The RAC Toolkit is user-friendly and has been developed and reviewed by experts in aged care and palliative care.
The RAC Toolkit has evidence-based resources on palliative care and advance care planning that can assist you and your organisation in meeting the Aged Care Quality Standards.

- Resources for clinicians and care workers, such as on providing grief and bereavement support.

Provides an opportunity for you to evaluate your learning and development needs and create a personal learning plan.

Assist members of your organisation to support palliative care and advance care planning for residents and their families and carers.
